Position Overview:

We are seeking a skilled and compassionate Home Health Registered Nurse (RN) to join our team. The ideal candidate will provide comprehensive nursing care to our clients ensuring their health and wellbeing. This role involves direct patient care coordination with other healthcare professionals and adherence to individualized care plans.

Key Responsibilities:

• Conduct initial and ongoing assessments of clients health status including physical emotional and social needs.
• Develop implement and evaluate individualized care plans in collaboration with clients families and interdisciplinary teams.
• Administer medications treatments and other nursing interventions as prescribed by physicians.
• Monitor and document clients health conditions and responses to treatments.
• Educate clients and their families on health maintenance disease prevention and safety protocols.
• Coordinate with healthcare providers therapists and other professionals to ensure comprehensive care.
• Maintain accurate and timely medical records in compliance with state and federal regulations.
• Provide emergency care and crisis intervention as needed.
• Participate in quality improvement initiatives and adhere to best practice standards.
• Ensure compliance with all HCS program requirements and policies.

Qualifications:

• Current and valid Registered Nurse (RN) license in the state of Texas.
• Minimum of 12 years of nursing experience preferably in home health or communitybased settings.
• Experience working with individuals with intellectual and developmental disabilities is highly desirable.
• Strong clinical skills and knowledge of nursing practices and procedures.
• Excellent communication interpersonal and organizational skills.
• Ability to work independently and as part of a multidisciplinary team.
• Compassionate patient and dedicated to providing highquality care.
• Valid drivers license and reliable transportation for travel to clients homes.
