RN Care Coordinator

Mobile Health Team Inc. a staffing agency has an opening for an RN Care Coordinator. We are looking for someone with a passion for people.

Location: Nome AK 99762

Assignment Start Date: 5/28/2024

13 weeks Varied 8hour shift 40 hours guaranteed weekly

Seeking an RN care coordinator for a primary care clinic in Bush Alaska. This position will coordinate care for patients in several remote locations for chronic care appointments followup for recent hospitalization and advocate for patient needs.

EXPERIENCE/EDUCATION/QUALIFICATION

Experience:

• Experience in Care Coordination/Case Manager required.

Licenses Certifications and/or Registration:

• Alaska is not a compact state the applicant must have a current and unencumbered RN license in the home state with an Alaska license application.

EMR System: Cerner knowledge/experience preferred

Shift: MF

Vaccinations: Current COVID and flu vaccines required.

Knowledge/Skills/Abilities required:

• Must be culturally sensitive respectful and courteous.
• Strong organizational skills communication skills with care providers and patients and computer proficiency and telephone communication are a must.

Housing: Housing will be provided with roommates so cannot have pets or S/O.
