Travel Physical Therapist

Join our healthcare client a rehabilitation facility providing postacute care located in Antioch CA:

• 8 OR 13 week contracts
• Schedule: Sunday Thursday OR Tuesday Saturday

Job Description:

• Develop and obtain approval for treatment plans from referring physician.
• Treat patients according to physicianprescribed plans.
• Assist nursing department in Restorative Aide training.
• Supervise Physical Therapy Assistants in patient care adhering to state regulations.
• Communicate patient progress and plans with team.
• Attend Patient Care Conferences and Rehabilitation Conferences as needed.
• Participate in staff inservices training.
• Document evaluations progress notes and recertification in compliance with PT Board regulations.
• Record treatment changes following policies.
• Provide family/caregiver training for discharge.
• Assist in discharge planning.
• Recommend and facilitate ordering of durable medical equipment for patient mobility.
• Report equipment issues promptly.
• Foster positive relationships with patients families and colleagues.

Qualifications:

• Valid Physical Therapist License
