Licensed Practical Nurse Home Healthcare

Requirements

We are currently seeking skilled and dedicated Licensed Practical Nurses (LPNs) to join our team. The LPN will be responsible for providing direct patient care and support to individuals in their homes ensuring their health and safety while promoting independence and quality of life.

Responsibilities:

1. Conduct comprehensive assessments of patients health status and needs in the home environment.
   
2. Develop and implement individualized care plans in collaboration with the patient their family and other healthcare professionals.
   
3. Provide skilled nursing care including medication administration wound care catheter care and other treatments as prescribed by the physician.
   
4. Monitor and evaluate patients progress and response to treatment adjusting care plans as necessary.
   
5. Educate patients and their families about their medical conditions treatments and selfcare techniques.
   
6. Assist patients with activities of daily living (ADLs) mobility and personal hygiene as needed.
   
7. Maintain accurate and uptodate documentation of patient assessments care plans interventions and outcomes.
   
8. Communicate effectively with the interdisciplinary healthcare team including physicians therapists and other caregivers to ensure coordinated and comprehensive care.
   
9. Provide emotional support and companionship to patients and their families fostering a supportive and caring environment.
   
10. Adhere to all regulatory standards policies and procedures governing home health care practice and patient confidentiality.
