SUMMARY
The primary purpose of the facilitybased APP is to administer Providerlevel direct patient care services within the PostAcute Care setting.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Embraces the philosophy of our company to render care in a manner approach and with the skill that promotes maintenance or enhancement of each patients clinical status and quality of life.
Conduct Annual Wellness Visits on all longterm Nursing Facility residents and may perform Annual History and Physicals as requested by Facilities.
Conduct clinical interventional and episodic direct care visits and appropriate reevaluation of care visits.
Provide ongoing evaluation and treatment of chronic comorbid illnesses as medically appropriate
Evaluate/treat residents for changes in condition.
Review medications orders and diagnostic tests and respond to clinical abnormalities appropriately for all patients under their care as allowed and/or requested by collaborating physicians.
Coordinate care with collaborating physicians as indicated for overall medical management.
Assist with facilitywide illnesses where appropriate collaborations exist.
Coordinate care with other disciplines such as Therapists Specialists and Social Workers.
Maintain communication with families/attend family conferences when possible.
Other duties as assigned
QUALIFICATIONS
Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field
Must possess a minimum of a Bachelors and Masters Degree in Nursing from accredited Colleges or Universities.
Experience in nursing is required and some experience as a Nurse Practitioner/Provider is preferred but not necessary although demonstrated critical thinking skills are required.
Must possess knowledge of Nursing and clinical Nurse Practitioner critical thinking skills application to the aged chronically ill and convalescent resident.
Strong leadership interpersonal and problemsolving skills preferred.
Must possess excellent written and oral communication skills
Must possess traits of punctuality organizational skills and ability to function within a team.

This position has a unique schedule and pay structure. This s a feeforservice healthcare company that compensates providers on a work Relative Value Unit (wRVU) system. Providers are paid semimonthly w/ Paycheck #1 compensating for the minimum required # of hours and # of patients per day; Paycheck #2 compensating for the # of vRVUs in excess of Paycheck #1.
APPs are required to work a minimum of 4 hours per day and see a minimum of 10 patients during this 4 hour period. 4 hours per day and 10 patients $90K per year minimum comp. 23 additional patients per day $23K additional pay per quarter. APPs average between $110120K per year on this system.
Hours of operation are 8:00 AM 6:00 PM (APPs must set schedule within these hours)
Oncall nights and/or weekends 34 times per month.
