The Project Director(s) shall be a proven organizational leader and digital transformation executive with at least seven (7) years of experience in the healthcare information field during the last ten (10) years including at least five (5) years of experience in leading performing or completing fullcycle health information transformation assessment activities.

The contractor shall identify describe map and visualize the current state of public health information practices data and tools in the context of syndromic surveillance electronic case reporting and public health information across departmental operations external relationships and the statewide public health information ecosystem.

The contractor shall identify describe map and visualize opportunities to streamline workflow optimize or integrate systems decrease point to point connections and leverage tools to improve data efficiency and effectiveness in support of current operations and future capability.

The contractor shall identify and describe data inputs sources storage analysis and dissemination across agency divisions and bureaus and units interdepartmental and interagency data relationships (inclusive of Department of Social Services Department of Mental Health and others as identified during discovery activities) the statewide public health ecosystem health information exchange and other stakeholders.

The contractor shall identify and describe interoperability opportunities and challenges across agency divisions and bureaus and units interdepartmental and others as identified during discovery activities the statewide public health ecosystem health information exchange and other stakeholders.

The contractor shall develop a mature map of data relationships to include capacity capability interoperability opportunities and challenges across agency divisions and bureaus and units intradepartmental the statewide public health ecosystem health information exchange and other stakeholders.

Required:

• 7 years Experience in Healthcare Information Field in the last 10 years

• 5 years Experience leading performing and completing full cycle health information transformation assessment activities
