• The Project Manager(s) shall be certified in project management with at least five (5) years of experience in the information management field during the last seven (7) years including at least three (3) years of experience in leading performing or completing health information transformation assessment activities.
• The contractor shall describe the current state strategic considerations and future opportunities across the state agency state and stakeholder public health information activities while identifying describing visualizing and scoring public health information practices tools and personnel in the context of syndromic surveillance electronic case reporting and public health information.
• The contractor shall propose to the state agency for acceptance one or more industryleading evidencebased internationally utilized digital health transformation and maturity frameworks previously leveraged at the state or federal level in North America or Europe for assessing the digital maturity of public or community health services agencies or organizations serving at least five million individuals.
• The contractor shall utilize the state agencyaccepted industryleading digital health transformation and maturity framework or frameworks to describe the current state strategic considerations future opportunities and digital maturity score across agency state and stakeholder public health information activities.
• The contractor shall implement the state agencyaccepted evidencebased framework and/or tool used to assess the status of digitally enabled public health or community care interoperability and sharing of information within and between jurisdictions at the local state and federal level.
• The contractor shall assess and describe the status development or enhancement of seamlessly integrated and interoperable health information exchange across stakeholders.
• The contractor shall assess and describe the status of data privacy and security measures to protect sensitive health information.
• The contractor shall assess and describe a history of reporting and benchmarking outcomes to assess programmatic effectiveness.
• The contractor shall evaluate describe and score all data capability and capacity elements.

Required:

• Project Management Certification
• 5 years experience in information management field in the last 7 years
