We are now seeking a Nurse Practitioner to join our Healthcare Group in the Primary Care division. The candidate will care/treat patients in a LongTerm Care facility in Glendale WI. Our clinicians are filled with high energy and excitement. We look forward to introducing you to the team!
Responsibilities:
- Examine patients and their medical records
- Prescribe medications
- Maintain accurate records and schedules
- Inform patients about their health conditions and prescribed medications
- Educate patients and their family members on diseases epidemics and prevention
- Oversee patients overall health
- Promote good health practices
- Advance professional knowledge and conduct research
- Propose treatment plan for Annual Wellness visits
Requirements:
- Candidate should possess a valid NP license in the state of Wisconsin
- Candidate should be OK with nursing home setting
Schedule:
- 3 days per week
- 8 hour shifts
