Requirements:
Active CA License
BC
BLS
DEA
With 3 years of experience in similar setting
PAs must have CA Controlled Substance Education Course Certificate
NPs must have Furnishing License with Schedules II
Immunizations
Flu Vaccine TB Status/XRay Results (results must be within the 90 days before start date)
COVID Vaccination proof
Primary series plus booster regardless of date received OR 1 dose of current COVID vaccine (2023/2024 formulation.
