Blanchard Valley Health System welcomes you to be part of our dedicated team serving patients in Northwest & Central Ohio. We are seeking a Lead CRNA to manage daily operations at our wellestablished practice.
Job Description:
Compensation and Benefits:
- Competitive salary range based on experience and training (to be discussed at interview). Provider may submit salary expectations with presentation.

Requirement description :
ALL OF THE FOLLOWING ARE REQUIRED AT NAME CLEAR UNLESS NOTED OTHERWISE.
- Board Certified CRNA REQUIRED
- OH license (or willing to obtain) REQUIRED
- At least 2 years work experience as CRNA REQUIRED
- Date of agencys last communication with provider (note in Additional Submission Details field) REQUIRED
- Why is candidate interested in BVHS (note in Additional Submission Details field) REQUIRED

- Board Certified
STATE LICENSE REQUIREMENTS :
- Ohio
