Hospice Case Manager RN

Mobile Health Team Inc. a staffing agency has an opening for a permanent fulltime RN Hospice Case Manager. We are looking for someone with a passion for people.

Location: Moab UT 84532

Assignment Start Date: 6/3/2024/ Flexible before July 2024

Anticipated Schedule: 67 x 12hour shifts per biweekly pay period and may include a mix of days nights and/or shifts. Willing and able to take 610 call shifts per month depending on staffing levels. Able to respond within 45 minutes response time.

Starting salary range: $64480.00 to $82534.00 annually DOE

A $10000 signing bonus is available with a 2year service requirement

This position identifies patient and family needs and coordinates care for terminally ill patients in their homes. This position works closely with physicians discharge planners charge nurses and other interdisciplinary team members. The right candidate is passionate about patient care and is comfortable working autonomously with patients and patient family members in the community. The position is salaried due to oncall responsibilities.

EXPERIENCE/EDUCATION/QUALIFICATION

Experience: Must have at least one year of recent RN experience.

Education: Must be a Graduate of an accredited school of nursing

Licenses Certifications and/or Registration:

• Current unencumbered Utah or compact state RN license.
• Current BLS certification.
• Valid Drivers License and reliable transportation for patient visits.

Knowledge/Abilities/Skills:

• Sensitivity to the needs of terminally ill patients and families as well as awareness of ones feelings about death and dying.
• Ability to independently provide a high standard of care in unstructured environments.
• Great customer service skills.
• Able to receive and manage sensitive information professionally and maintain confidentiality at all times.
• Able to handle stressful situations calmly; emotionally mature.
• Reliable and timely with work schedule.
• Flexibility and willingness to accept other duties as assigned.
