Program Specialist Social Worker

Position: Program Specialist (Social Worker) 1

Duration: 12 Months

Shift Time: 8AM 5PM (MonFri)

Location: Remote (Texas)

Note:

• Remote role Will require facetoface training at the office twice per year.
• Work independently from home.
• Written medical/ social documentation.
• High speed internet.

Professional Summary:

• Conduct screening and risk assessment interventions per program guidelines.
• Assist with nonmedical aspects of the members care including referrals to community resources.

Job Responsibilities:

• Identify special needs members through the completion of health screens and other resources Work with community outreach/member advocates to coordinate member care Educate providers and community resources on program components and available support services.
• Educate members with special needs to foster compliance with program and positively impact outcomes.
• Members contact via phone to follow up with requested items and services.
• Contacting physicians DME agencies and service providers and documenting detailed contact in the electronic member records.
• Review of documented forms for errors and request corrections or clarifications from assessing staff.
• Members must be contacted within 5 days of the ISP start date and then as directed by the work process.
• All turnaround times are short within 15 business days expectation is to complete within posted tats.
• Conduct site visits as appropriate for programs and provide support to other special programs.
• Develop and modify care plans in conjunction with members members family and managing physician.
• Development of plan specific literature and education materials in conjunction with medical director and corporate oversight.

Education/Experience:

• Bachelors degree in Social Work Nursing Health Behavioral Science or Equivalent Experience.
• Experience in a managed care environment preferred. 02 years of experience
