PT - Outpatient - TherapyRehabilitation

• Shift: Day 5x8Hour (08:00 16:30) 40hrs per week
  
• Location: Onsite; Grantsburg WI
  

• Conduct comprehensive assessments to evaluate patients physical conditions mobility strength range of motion and functional abilities.
  
• Develop individualized treatment plans based on the assessment findings patient goals and the nature of the injury or condition.
  
• Administer a variety of therapeutic interventions including therapeutic exercises manual therapy joint mobilization neuromuscular reeducation and modalities (such as ultrasound or electrical stimulation).
  
• Educate patients about their conditions treatment plans and home exercise programs to empower them in managing their health and achieving optimal outcomes.
  
• Address and manage pain through various therapeutic techniques exercises and modalities.
  
• Regularly assess and reassess patients progress to modify treatment plans as needed and ensure the achievement of rehabilitation goals.
  
• Maintain accurate and detailed patient records documenting evaluations treatment sessions progress notes and any changes in the treatment plan.
  
• Recommend and provide training on the use of assistive devices such as crutches walkers or braces to improve mobility and independence.
  
• Advocate for patients needs and collaborate with them to set realistic and achievable rehabilitation goals.
  

Requirements

• Bachelor s or higher
  
• Graduation from an approved Physical Therapy curriculum
  

• PT Physical Therapist
  
• BLS Basic Life Support
  

• At least 1 year of experience required prefer more
