Job Description
Are you a dedicated RN looking to make a difference in the lives of veterans Were seeking an RN Case Manager to join our team and work collaboratively with the PCMHI team to provide comprehensive care to our patients. As an RN Case Manager youll be responsible for conducting basic BHL baseline measured care assessments/structured interviews patient care chart reviews 5pt triage assessments and determining a course of action related to the PCMHI protocol (developing a treatment plan).
