- Active OH license REQUIRED please do not submit providers needing to obtain license
- ACLS Certified REQUIRED
- Flu vaccine REQUIRED at time of credentialing
- COVID Vaccine Preferred
- 100% CLEAN BACKGROUIND REQUIRED
