As the SNFist you will act as a Skilled Nursing Facility s leading provider of internal medicine services
You will collaborate with the facility s interdisciplinary team and physicians serving as a daytoday clinical leader while helping oversee the care of all of the facility s residents
You will work with the entire facility s patient population short and longterm residents overseeing their daytoday care with the assistance of the facility s nursing staff and guidance from the facility s medical directors and attending physicians
Evaluation of newadmission visits
Coordination of discharge visits
Monthly stability visits for long term residents
Evaluations for patients exhibiting changes in condition
As a SNFist you will provide types of care including but not limited to:
Advanced Care Planning
Provide comprehensive health assessments differential diagnosis and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
Prescribe all necessary medications and treatments
Perform comprehensive physical examinations of patients
Develop and implement patient management policies and procedures
Facilitate referrals to other healthcare professionals and medical facilities
Requirements
- Advanced Practice License (APN) with a NY license
- Must have an active license and DEA number.
- Previous experience in nursing or other medical fields.
- Ability to build rapport with patients and their loved ones.
- Ability to thrive in a fastpaced environment.
- Excellent written and verbal communication skills.
- Strong leadership experience with emphasis on teamwork in a healthcare setting.
- Master s degree required
