Care Assistant

As a Domiciliary Care Worker specializing in Complex Care you will play a crucial role in delivering exceptional care to individuals with complex health conditions within their own homes. Working closely with healthcare professionals and our dedicated team you will contribute to enhancing the quality of life for our clients promoting independence and ensuring their wellbeing.

Responsibilities:

1. ClientCentered Care:
   

   • Provide personalized care plans tailored to the unique needs of clients with complex health conditions.
     
   • Administer medication perform clinical procedures and assist with medical equipment as required.
     

2. Assessment and Monitoring:
   

   • Conduct comprehensive assessments of clients health needs and regularly review care plans in collaboration with the multidisciplinary team.
     
   • Monitor and record vital signs symptoms and any changes in clients conditions reporting findings promptly.
     

3. Communication and Collaboration:
   

   • Liaise with healthcare professionals including nurses therapists and physicians to coordinate and implement effective care strategies.
     
   • Communicate effectively with clients their families and other care providers to ensure continuity of care.
     

4. Promoting Independence:
   

   • Encourage and support clients in maintaining their independence providing assistance where necessary without compromising their dignity.
     

5. Documentation:
   

   • Maintain accurate and uptodate records of care provided including observations interventions and client responses.
     

6. Emergency Response:
   

   • Be prepared to respond calmly and efficiently to emergency situations following established protocols and seeking immediate assistance when necessary.
     

7. Professional Development:
   

   • Engage in ongoing training and development to stay abreast of the latest advancements in complex care and related healthcare practices.
     

Requirements

• NVQ Level 2 or 3 in Health and Social Care (or equivalent).
  
• Previous experience in providing care for individuals with complex health conditions.
  
• Knowledge of medical conditions medications and relevant healthcare protocols.
  
• Strong communication and interpersonal skills.
  
• Compassionate patient and dedicated to delivering highquality care.
  
• Ability to work independently and as part of a team.
