Job Descriptions:
At a minimum of 13 weeks, could be extended depending on staffing and ability to recruit full time.
40 hrs/wk
Social Worker discharge planner needed to work with various medical populations.
Must have recent 2 years of acute hospital discharge experience, masters required. No behavioural health. Only hospital adult discharge planning experienced SW need apply.
Will coordinate with RN case managers and other medical team members to coordinate safe discharge plans. Fast-paced units. hours 8-430p.
Must have LMSW with recent hospital experience.
