JOB DETAILS

• Shifts Available: Days- 8 hours. 40 hours/week, 8am - 5pm; Monday - Fridays
  
• Work location: Onsite; Pleasanton, CA
  

JOB DESCRIPTION

• Act as advocates for patients, ensuring that their needs and concerns are communicated to the healthcare team. They may help patients navigate the healthcare system and understand their treatment plans.
  
• Coordinators work with physicians, nurses, and other healthcare providers to develop and implement individualized care plans for patients. This involves assessing patients needs, setting goals, and coordinating the delivery of care services.
  
• Liaise between different healthcare professionals, departments, and patients to facilitate smooth and efficient care. They may also educate patients and their families about treatment plans and self-care.
  
• Manage appointment schedules, ensuring that patients receive necessary tests, treatments, and follow-up care. They may coordinate with various departments to schedule surgeries, consultations, and other healthcare services.
  
• Maintain detailed records of patient assessments, care plans, and coordination efforts. This helps in tracking progress, evaluating outcomes, and providing continuity of care.
  
• Play a vital role in coordinating rapid responses and ensuring that appropriate interventions are implemented.
  

Requirements

MINIMUM QUALIFICATIONS

Education Qualifications

• Bachelor s Degree or higher in Nursing from an accredited academic institution.
  

Licenses and Certifications

• RN - Registered Nurse
  
• BLS - Basic Life Support
  

Experience

• 2 years experience as registered nurse, with a preference for candidates who have prior clinical experience in dermatology or oncology
