We are seeking a dedicated and experienced Registered Nurse (RN) in Tucson, AZ, as a Care Manager. As a Care Manager RN, you will play a pivotal role in creating and administering care plans, focusing on the prevention and management of kidney-related health issues. This is not a direct clinical services role but involves comprehensive care coordination and support for patients, particularly those with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD).

Work schedule: Monday to Friday, 8 AM - 5 PM, with 3-4 days on the road visiting patients and 1-2 days telehealth.



Requirements

  • Graduate of an accredited School of Nursing.
  • Current RN licensure in the State of Arizona or a Nurse Licensure Compact (NLC) state.
  • Minimum of 2 years of previous experience in care management and/or working with CKD/ESRD patients preferred.
  • Home Health and/or Hospice experience is required.
  • Ability to travel within a one-hour radius for in-home visits.
  • Strong communication and interpersonal skills.

High School diploma or GED. Minimum of 2 years of direct patient care experience. Valid Registered Nurse (RN) license/certification in the state of Texas (TX). Must be available for 10 consecutive days of training, Monday to Friday, 8:00 AM to 5:00 PM. Availability for weekend shifts, which may be scheduled from 7:00 AM to 3:00 PM, 3:00 PM to 11:00 PM, or 7:00 AM to 11:00 PM. Valid American Red Cross Basic Life Support (ARC BLS) certification. Valid Regular Driver License (DL). Valid American Heart Association Basic Life Support (AHA BLS) certification.
