Summary:
One of our exclusive partners is seeking (3) Care Managers to join their team ASAP! These are permanent jobs, and are based out of The Bronx, NY. Candidates must have at least 1+ years of Case Planning, Case Management, and pediatric experience to be considered. Please make sure this reflects on your resume.
Responsibilities:
- Monitor and evaluate patient's needs
- Obtain enrollment consents from individual or legal guardian
- Responsible for Individual Plan of Care
- Face-to-face client visits
- Maintain and write documentation on patient progress
Requirements:
- Bachelor's Degree in Human Services (psychology, sociology, etc.)
- Master's Degree preferred (offers higher pay)
- 1+ year of Case Management/Care Management exp
- Pediatric exp preferred
- Must be Bilingual Spanish
