Job Description

Key Responsibilities:

  1. Patient Assessment: Conduct thorough assessments of residents health, including vital signs, medical history, and current conditions. Regularly monitor and document changes in residents health status.
  2. Medication Administration: Administer prescribed medications and treatments accurately, following facility protocols and ensuring residents safety. Document medication administration and report any adverse reactions.
  3. Wound Care: Provide wound care, dressing changes, and manage other medical equipment as needed for residents with chronic conditions or wounds.
  4. Assistance with Activities of Daily Living (ADLs): Assist residents with bathing, dressing, grooming, toileting, and mobility as required. Encourage and support residents independence when possible.
  5. Healthcare Planning: Contribute to the development and implementation of individualized care plans for residents in collaboration with registered nurses (RNs) and other healthcare professionals.
  6. Monitoring and Reporting: Continuously monitor residents conditions, promptly report any changes or concerns to the RN or healthcare provider, and document all care provided accurately.
  7. Infection Control: Follow strict infection control procedures to prevent the spread of infections within the facility. Educate residents and staff on infection prevention.
  8. Communication: Maintain open and effective communication with residents, their families, and other healthcare team members. Provide emotional support to residents and their families during difficult times.
  9. Safety Measures: Ensure residents safety by identifying and mitigating potential hazards, using proper lifting and transfer techniques, and responding to emergencies appropriately.
  10. Documentation: Maintain accurate and up-to-date medical records, including nursing assessments, care plans, and incident reports.
  11. Education: Stay current with nursing best practices and participate in ongoing training and educational programs to improve skills and knowledge.
  12. Compliance: Adhere to all relevant state and federal regulations, facility policies, and nursing standards of practice.


Requirements

State license requirement: NE

CPR and Basic Life Support (BLS) certification.

Strong communication and interpersonal skills.
