Job Description

JOB DETAILS:
  • Shifts available: 8am-4:30pm. 40hr/week
  • How many open positions: 2 positions (1 is a float Dietitian, between San Jose and San Francisco)
  • Target Start Date: ASAP
  • Work Location: San Francisco, CA

Under the administrative direction of the Program Manager and in consultation and supervision with the Dietitian Specialist, the Dietitian will coordinate the dietary services of Day Health Center and offer direct services to Company participants and allied health professional team members.


Duties and Responsibilities
  • Visits and observes participants in order to evaluate their nutritional needs. Reviews diet orders in charts and confer with physicians, nurses, and participants family, when necessary. Completes dietary assessment and quarterly progress note for participants as required.
  • Works with and participates actively in the interdisciplinary team regarding diet and nutritional problems and management, including participant care plans, problem solving and case review. In collaboration with Center/Program manager and other specialists promotes staff development in the area of nutrition.
  • Provides nutritional education to participants, family and staff as needed; coordinates use of community resources. Provides information to the Nutrition Services Manager concerning preparation of meals for participants of the Day Health Center including menu planning, diet textures, and nutritional planning.
  • Fulfills all requirements, such as reporting, keeping statistical records, making entries into charts and attending meetings as required.

Requirements

Education
  • Graduate of an approved Dietetic program.

Licenses and Certifications
  • Registered by the Commission on Dietetic Registration.
  • California Drivers License and willing to drive and or travel for participant visitation.

Experience
  • Minimum of two years as a Registered Dietitian in a clinical setting with experience working with the frail elderly or geriatric population required.

Languages (preferred, not required)
  • Spanish
  • Cantonese
  • Mandarin

KNOWLEDGE/SKILLS & ABILITIES:
  • Ability to work independently, as well as with the interdisciplinary team.
  • Experience in leadership and training.
  • Flexible, respectful, resourceful, self-motivated, and professional.
  • Bilingual ability may be required for specific assignments.
