Job Summary

Social Worker - Saskatoon

Idylwyld Medical Centre is seeking a Temporary Full Time Social Worker. Reporting to Dr. Shawn Smit or Renee Jansen the Social Worker will be responsible for case management assessment and counseling services to adults who are experiencing changes in their quality of life due to the deterioration of their mental health and/or social circumstances. Working collaboratively with a number of MHAS providers and the community the primary responsibility of the incumbent will be to provide assertive outreach case management services in a variety of community settings to a wide range of adult clients. The incumbent will work as part of a team focused on providing psychosocial case management supports to persons in need of assistance with understanding their illness recovery planning housing ongoing engagement and assessment coordination of services and supports and managing crisis as relevant. Depending on client demand the incumbent may be assigned to provide counseling supports to other MHAS Teams as required.

If you have a Bachelor of Social Work (BSW) and are Registered/eligible with the Saskatchewan Association of Social Workers (SASW) with experience in the area of mental health and addiction services and supervised clinical counselling in a primary health clinic then this job is for you!

This position is part of a newly developed Innovation Fund between the Saskatchewan Government and medical clinics across Saskatchewan with the aim of improving patient access and quality care through team-based support that will deliver better access to primary care in the province of Saskatchewan. This position is temporary until July 31 2027 with the possibility of an extension.

Responsibilities:

  • Within the school/community context will complete social emotional and behavioural assessments of children (typically 3 8 years of age) provide consultation and counselling as identified by community partners
  • Maintain regular and frequent contact with family members and community partner personnel in homes schools and other community settings as appropriate
  • Provide concrete support for families (e.g. housing transportation)
  • Provide coordination of appropriate referrals for specialized services for children and their families
  • Crisis intervention for a wide variety of clients special needs which must be addressed immediately (e.g. suicide risk assessment)
  • Behavior management activities with selected individuals or families as needed Consultation
  • Ensure effective working relationships with other professionals
  • Liaise on an ongoing basis with key community teams
  • Case conferences with families and agencies
  • Attend regular team meetings huddles and conferences with professional coworkers Communication
  • Prepare and maintain clinical reports reviews and other records as required by Child and Youth Program Mental Health and Addictions Services
  • Maintain appropriate communication with partners (i.e. school staff childs family and other agencies involved with child and family).
  • Set up and chair meetings as needed
  • General administrative duties as needed
  • Collaborate with team members on monitoring evaluation and quality improvement efforts
  • Participate in engagement of clients and families as advisors for continuous improvement

Qualifications:

  • Bachelor of Social Work (BSW)
  • Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
  • Experience and training in Motivational Interviewing Solution-Focused Counseling and

Cognitive Behavioral Therapy.

  • Valid Saskatchewan Drivers License and reliable transportation
  • 1 year experience in the area of mental health and addiction services and working with mental health and addictions clients
  • 1 year of supervised clinical counselling experience
  • Demonstrated competence in providing clinical assessments and intervention services for children and their families
  • Knowledge of child development family systems theory addiction issues mental health disorders would be beneficial
  • Knowledge of child development and positive parenting strategies
  • Demonstrated cultural competence and knowledge of First Nations family systems and cultural values and practices within the community
  • Knowledge of and established working relationship with Core/Downtown community agencies
  • Understanding of the Ministry of Social Services policies procedures regulations and plan manual
  • Knowledge in navigating health and child protection systems
  • Strong knowledge and understanding of Saskatchewan First Nations and Métis history culture and spirituality including protocols; in-depth Trauma-informed practice
  • Knowledge of the TRC Calls to Action and ability to implement recommendations in practice with an understanding the impacts of colonization residential school 60s scoop has had on individuals and families with demonstrated application in practice
  • Knowledge in various mental disorders etiology treatment and intervention
  • Knowledge in counseling principles practice theory and intervention strategies
  • Demonstrated knowledge and ability for competent case management practice
  • Knowledge in marginalized populations
  • Excellent verbal and written communication skills. Knowledge of an Indigenous language would be considered an asset
  • Successful experience working collaboratively with an interdisciplinary team community agencies and professionals
  • Strong organizational and leadership skills
  • Knowledge of cross-cultural issues and cultural sensitivity
  • Advanced interpersonal skills
  • Strong computer skills. Previous experience with Med Access EMR would be a benefit.
