Circle Medical Centre is seeking a Temporary Part Time Social Worker. Reporting to the Supervising Physician the Social Worker (BSW/MSW) will be responsible for case management advocacy resource navigation and trauma-informed counselling to support individuals and families facing social emotional mental health and practical challenges. Working as part of an interdisciplinary team this role focuses on delivering affirming client-centred care to vulnerable and underserved populations including members of the LGBTQ2S community people who are unhoused and those living with addictions.

This position is part of a newly developed Innovation Fund between the Saskatchewan Government and medical clinics across Saskatchewan with the aim of improving patient access and quality care through team-based support that will deliver better access to primary care in the province of Saskatchewan. This position is temporary until March 31 2027 with the possibility of an extension.
Responsibilities:
Provide assessment counselling crisis intervention and ongoing support to clients experiencing mental health trauma addiction housing and other complex psychosocial challenges
Develop client-centred care plans and build therapeutic relationships using strength-based and harm reduction approaches
Advocate for clients and connect them with community healthcare housing financial and social supports
Support vulnerable and underserved populations including LGBTQ2S clients individuals experiencing homelessness and those facing stigma discrimination or social isolation
Collaborate with the interdisciplinary team participate in case conferences and maintain accurate clinical documentation
Promote inclusive culturally responsive care and support clients and families through life transitions grief and perinatal or postpartum needs as applicable
Qualifications:
Bachelor of Social Work (BSW) required; Masters of Social Work (MSW) is an asset
Registered and in good standing with the Saskatchewan Association of Social Workers (SASW)
13 years of experience in healthcare community-based care mental health or social services
Experience supporting vulnerable and underserved populations; LGBTQ2S and gender-diverse client experience is an asset
Strong skills in assessment behavioural counselling crisis intervention case management advocacy and resource navigation
Knowledge of trauma-informed harm reduction equity-focused and culturally responsive practice
Strong communication documentation organization and collaboration skills in a multidisciplinary team environment
