RESPONSIBILITIES

This employee will work within a 24-hour residential housing model that will serve a total of 10 individuals in partnership with another Community Agency Service Provider. Individuals served will have a confirmed Mental Health Diagnosis & Intellectual Developmental Disability. They will provide delivery of direct support services to all residents within the housing apartment model and ensure the physical and emotional well-being and safety/security of all individuals within the residential site in alignment with YWCA Hamiltons Mission and Vision Statements and in cooperation with partnership of Community Agency Service Provider.

KEY RESPONSIBILITIES:

This is a unique partnership with another Community Service Agency and funding from two Ministry streams in order to provide the appropriate services and supports to meet the complex needs of the residents.

The employee will work in cooperation and alongside with other Community Agency Staff in cooperation YWCA Hamilton Staff including Assigned Program Supervisor full time part-time and relief team members.

They will perform all duties and responsibilities in a productive professional and cooperative manner which will contribute to the overall goals of the department and in keeping with general policies and guidelines of YWCA Hamilton.

Ensure the physical and emotional well-being of individuals we support is maintained. Meet QAM Compliance and MCSS Legislation 299/10 Standards.

Follow and adhere to QAM compliance and Legislation 299/10 standards.

This employee will provide delivery of direct support services to individuals we support within their residential housing model.

Support services in all areas of daily living within our service plan domains including but not limited to: bathing hygiene routines medication administration meal preparation grocery shopping chore lists/housekeeping laundry nightly safety inspections and in-house recreation opportunities and community outings that promote social inclusion.

Provide supervision and ensure safety of residents during their shift.

Support services offered will maintain or increase individuals independent living skills and personal growth.

OTHER RESPONSIBILITIES:

In cooperation with program supervisor will perform all duties and responsibilities in a productive professional and cooperative manner which will contribute to the overall goals of the partnership and DS Department and in keeping with general policies and guidelines of YWCA Hamilton

Ensure the physical and emotional well-being of individuals we support is maintained

Assist residents with ADLs meal preparation grocery shopping menu planning medication administration assistance with hygiene routines housekeeping laundry community safety skills transportation reducing risks to harm community outings recreation and social opportunities

Implement and carry-out individual program plans Behaviour Support Plans and protocols

Must attend and participate in regular team meetings in order to communicate and share information including achievements progress concerns and generate solutions

Attend on-going professional development opportunities offered to program staff

Contribute in the development planning implementation of day/evening and weekend in-house and community based social/recreational activities

Program and agency policies are adhered to by staff and reviewed regularly

All necessary administrative/documentation tasks are performed including: MAR sheets daily communication logs checklists chore lists inspections incident reports or other reports/tasks as assigned

Ensure that housekeeping/cleanliness/maintenance of program areas or residents accommodations standards are met

Establish positive professional relationships with staff individuals families and other community service agency providers

Ensure YWCA Hamilton workplace health and safety policies and procedures are followed at all times and act to reduce workplace hazards.

Report all workplace hazards and follow YWCA Hamilton health and safety policies

QUALIFICATIONS

Minimum Education: SSW or enrolled for completion DSW Apprenticeship or enrolled CSW or other human service field

Minimum 1-2 Years Part Time or Relief work experience in a 24hr residential setting

Previous work experience with individuals with Mental Health Diagnosis and Challenging Behaviours

Emergency First Aid/CPR Certification

Mental Health First Aid Certification

NVCI Certification

QAM/Abuse Training Fire Safety

ARAO Training

Must have demonstrated excellent verbal and written skills

Ability to work independently and as a part of the team

Must be able to work all the rotating schedules as assigned (see schedule)

Proficiency with MS Office Applications including Word & Outlook

Working from a feminist anti-oppressive/anti-racist perspective

Karen (Burmese) & French Language Skills an Asset

Must provide a current Vulnerable Sector Police Check or willingness to obtain at own cost

Attend and participate in all YWCA Hamilton mandatory training

Must be willing to provide a pre- employment medical examination and annually thereafter
