Job Summary
Within the Department of Family Medicine the Queens Family Health Team (QFHT) is a multidisciplinary team of health care professionals who provide primary health care services to the Kingston community. As an academic teaching centre the Department trains future family physicians and conducts research in issues of primary health care delivery. QFHT is comprised of a number of medical practices and provides for a diverse client centred work environment for all its employees. QFHT provides primary health services to rostered patients while modeling safe efficient and collaborative clinical processes to Family Medicine residents and other health professional learners.
Reporting to the Clinic Manager administratively and the Lead Physician clinically the incumbent will practice patient-centred care to achieve optimal drug therapy outcomes for enrolled patients as part of a team of primary care health professionals. The incumbent will provide pharmaceutical care to patients by optimizing medication therapy and acting as a resource to the team. The incumbent will provide regular educational updates with respect to drug-related guidelines and evidence-based research to patients physicians residents and other professional staff within the clinic.
In partnership with patients and other health care providers the Pharmacist uses unique knowledge and skills to prevent or resolve actual and potential pharmaceutical-related problems and ensures that the pharmaceutical needs of the patients are met.

Communicates with hospital and community pharmacists and other interdisciplinary team members to ensure a smooth transition for medication related care between care sites. Performs medication assessments/counselling for: o Older patients with chronic diseases or chronic medications; o Patients with multiple medications with an objective of appropriate dosage scheduling and to manage or prevent drug related problems; o Recently hospitalized patients who have had medication changes. Provides clinic team members with current information and guidelines on new drugs contraindications therapeutic use of drugs and other information to assist In the provision of quality evidence-based care to patients. Contributes to the education of family medicine residents/learners regarding medication safety and reconciliation. Provides patient education through individual or group sessions to help In the understanding and management of health issues using pharmacy and therapeutic agents. Develops/revises policies and procedures related to medication use; participates in the development of medical directives. Conducts patient home visits when appropriate. Records and documents care provided within the patients electronic medical record. Maintains confidentiality in all aspects of patient staff and organization information. Maintains and develops professional competence through ongoing professional development. Participates in QFHT committees and program leadership eg. Chairs Pharmacy & Therapeutics Committee member of Quality Improvement Committee. Undertakes other duties or special projects as may be assigned In support of the QFHT.
REQUIRED QUALIFICATIONS: Bachelor of Science in Pharmacy from a recognized university. Registered and in good standing Part A Ontario College of Pharmacists. Community or hospital pharmacy residency completion or equivalent clinical experience. Additional training or credentialing in pharmacotherapy (eg. Certified Geriatric Pharmacist Certified Diabetes Educator Certified Asthma Educator etc.) considered an asset. Additional training such as the Structured Practical Experience Program Teaching Associate Educational Program (University of Toronto) and in Evidence-based medicine principles considered an asset. A minimum of 5 years practice experience required to demonstrate competencies. Teaching experience will be considered an asset.
SPECIAL SKILLS: Demonstrated clinical skills in assessment program planning implementation and evaluation. Excellent verbal and written communication skills; outstanding interpersonal skills. Ability to work both independently and effectively in an interdisciplinary team environment. Understanding of community resources. Excellent organization time management and priority setting skills.
