This opportunity is with our Good Shepherd Seniors at Risk Program

Good Shepherd Centres Hamilton a ministry of the Hospitaller Order of St. John of God strives to enhance the quality of life for all who seek assistance and support in our community. The Seniors at Risk in Hamilton (SaRiH) program supports vulnerable seniors by helping them access essential services resources and community supports while promoting health safety wellness and independence.

How You Will Make an Impact

In this role you will provide individualized home and community-based support to vulnerable seniors and work collaboratively with clients care teams and community partners. You will help clients identify their needs and goals develop care plans access community resources and overcome barriers to care.

Services include case management referrals advocacy system navigation crisis prevention and intervention accompaniment to appointments and community activities and support with activities of daily living. The successful candidate will demonstrate a commitment to providing services in a dignified culturally competent anti-oppressive and respectful manner.

Responsible for participating in Good Shepherds Health and Safety programs procedures procedures and work in compliance with the provisions of the Occupational Health and Safety Act and its regulations to minimize risk to clients staff and others as well as to the property of Good Shepherd.

What Youll Bring to Our Team

  • A post-secondary degree/diploma in Social Work Social Services or a related field or an equivalent combination of education and experience.
  • Minimum of 3 years of relevant experience in human and/or health services.
  • Knowledge of case management assessment crisis intervention harm reduction trauma-informed care anti-oppressive practice and motivational interviewing.
  • Extensive knowledge of community resources and services social and environmental determinants of health and community development.
  • Excellent knowledge of provincial legislation and processes to support persons who are victims of abuse including the Office of the Public Guardian & Trustee
  • Excellent verbal and written communication interpersonal organizational and computer skills.
  • Demonstrated ability to work effectively as a member of a multidisciplinary team.
  • CPR/First Aid certification.
  • Valid Ontario drivers license and access to a vehicle insured for work purposes are required.
  • Committed to anti-racist/anti-oppressive practice and to the mission and values of Good Shepherd.
  • Fluency in a language in addition to English is an asset.
  • Demonstrated understanding of workplace health and safety practices and an employees responsibility under current legislation.
  • Proficient in computer software such as Word Excel and PowerPoint.
  • Good organizational skills and ability to prioritize workload.
  • Demonstrated commitment to professional continuing education.
