Job Summary
The Chronic Thromboembolic Pulmonary Hypertension (CTEPH) Program Nurse Coordinator is responsible for coordinating and supporting the comprehensive care of patients with suspected or confirmed CTEPH throughout the diagnostic treatment and follow-up continuum. The Nurse Coordinator serves as the primary point of contact for patients caregivers referring physicians and the multidisciplinary CTEPH team ensuring timely access to specialized care and the seamless coordination of diagnostic investigations treatment planning patient education and longitudinal follow-up.
Working closely with thoracic surgeons respirologists interventional radiology and administrative staff the Nurse Coordinator supports clinical excellence quality improvement initiatives and program development activities.
Duties
- Coordinate the assessment investigation and management of patients referred to the CTEPH program.
- Triage referrals and facilitate timely consultation appointments.
- Coordinate diagnostic testing including:
- Ventilation-perfusion (V/Q) scans
- CT pulmonary angiography
- Echocardiography
- Right heart catheterization
- Pulmonary function testing
- Exercise testing and other investigations as required
- Monitor patient progress throughout the diagnostic and treatment pathway.
- Facilitate multidisciplinary case reviews and treatment planning discussions.
- Coordinate transitions of care between inpatient outpatient and community settings.
- Serve as the primary contact for patients and families within the CTEPH program.
- Provide education regarding:
- CTEPH diagnosis and disease process
- Diagnostic procedures
- Anticoagulation management
- Pulmonary hypertension therapies
- Balloon pulmonary angioplasty (BPA)
- Pulmonary thromboendarterectomy (PTE) surgery
- Ongoing monitoring and self-management
- Assess patient needs and identify barriers to care.
- Provide emotional support and connect patients with appropriate resources and support services.
- Promote patient engagement and adherence to treatment plans.
- Collaborate closely with physicians NPs cardiology interventional radiology and administrative personnel.
- Participate in multidisciplinary clinics rounds and case conferences.
- Liaise with referring physicians and external healthcare providers to ensure continuity of care.
- Coordinate referrals to internal and external specialists as required.
- Support clinical research activities and patient recruitment for approved studies.
- Contribute to data collection reporting and outcome monitoring.
- Participate in program evaluation and quality improvement projects.
- Assist in meeting accreditation and reporting requirements.
Qualifications :
- Completion of a university college or other approved nursing program
- Current Certificate of Registration with the College of Nurses of Ontario (CNO)
- Current Basic Cardiac Life Support (B.C.L.S.) program Level II: Basic Rescuer certification
- Bachelors Degree in Nursing required; Masters Degree preferred
- Relevant clinical experience preferably in respirology cardiology ICU transplant or other advanced practice areas preferred
- Coronary Care I - requred
- Physical Health Assessment Course preferred
- Completion of the ECG interpretation course is an asset
- Demonstrated ability to exercise initiative and good judgement with the ability to multi-task
