Job Summary
In accordance with the Vision Purpose and Values and strategic direction of the Vancouver Island Health Authority (Island Health) patient and staff safety is a priority and a responsibility shared by everyone; as such the requirement to continuously improve quality and safety is inherent in all aspects of this position.
To serve as a single entry point able to screen and accept referrals for a variety of Community Programs provide consultation regarding program selection organize appropriate program intervention and maximize the efficient use of Community manpower.
Under the direction of the Manager uses independence and initiative to screen plan and coordinate care for clients in Community settings. The Coordinator functions as a Community representative for Long Term Care Home Nursing Care Quick Response Team VISTA Elder Abuse and other services projects and programs as required; understanding of these programs to healthcare professionals clients and families facilitating referrals participating in program manpower management and relaying accurate client information to appropriate health care professionals.
Eligible to practice in British Columbia under the Registered Nurses Act. Have a minimum three years experience in a health related field (community based experience an asset). Be computer literate and capable of data entry.
- Ability to communicate effectively verbally and in writing.
- Possess a comprehensive knowledge of Community support services.
- Ability to work independently and prioritize the workload keep professional judgment empathy tact and integrity.
- Ability to communicate effectively and deal with staff other health care professionals and the public.
- Must be flexible adaptable and able to accept change as the program evolves.
