Job Summary:
As a Case Manager Behavioural Support you will be part of a Behaviour Support Team that providesdirect care education and support to seniors who are demonstrating responsive behaviours that may beassociated with various types of dementia delirium mental health addictions or other neurologicalconditions and who are ALC or at risk of becoming ALC in the acute care settings of the Ontario HealthCentral or Toronto Central area. You will assist in the creation and coordination of a behaviour supportnetwork you will provide direct service to support the person served family members and/or care- giversto mitigate escalating responsive behaviours. A key role of this position is to coordinate and support thetransition of person served as they moved to their next experience.
The Behaviour Support Team provides direct care education and support to older adults and their care partners who are demonstratingresponsive behaviours/personal expressions that may be associated with various types of dementiamental health substance use or other neurological conditions who are designated ALC or at-risk ofbecoming ALC in hospital settings. The team works within hospitals in Toronto York Region andSouth Simcoe and completes a comprehensive behaviour support assessment and creates a person-centered Behavioural Support Plan. Transition support is provided to those individuals who are beingdischarged from hospital to their next care destination. Serving over 700 individuals per year theBehaviour Support Transition Resource Team plays a key role in supporting hospital staff in themanagement of responsive behaviours/personal expressions.
The @Home Behaviour Support team provides direct care education and support to older adults and their care partners who are demonstrating responsive behaviours/personal expressions that may be associated with various types of dementia mental health substance use or other neurological conditions who are being discharged from hospital within North Toronto York Region and South Simcoe. The team completes a comprehensive behaviour support assessment and creates a person-centered Behavioural Support Plan. Working as a member of an integrated team along side other home and community-care providers the @Home team supports the transition from hospital and works within the individuals own home.
What You Will Do:
- Assists clients in the development and implementation of a Behavioural Support Plan tailored tothe clients needs and issues designed to improve quality of life and assist in the mitigation ofresponsive behaviours. Identify and refer to resources specific to those needs and issues;
- Assists persons served in the development and implementation of a Behavioural Support Plantailored to the needs of the person served and issues designed to improve quality of life and assistin the mitigation of responsive behaviours. Identify and refer to resources specific to those needsand issues;
- Act as a liaison and advocate for the person served among those involved with the individualscare including family other care-givers medical and other multi-disciplinary providers;
- Complete Behavioural Support Assessment and provide timely and accurate record keeping andreports as required;
- Provide support on-going training and consultation to colleagues family members and/or othercare-givers to assist in mitigating escalating responsive behaviours;
- Provide supportive counselling or referral and linkages to health care programs or service
providers; - Provide crisis management/intervention and assist in the development of crisis plans for personsserved;
- Develop respectful and trusting relationships with persons served using Recovery and
Psychosocial Rehabilitation principles; - Maintain strict confidentiality and follow all other required policies and procedures;
- Effectively participate as a member of the Behaviour Support Resource Team; attend staff/teammeetings care conferences for the person served other multi-disciplinary meetings committeemeetings etc as required;
- Attend and actively participate in all required training;
- Liaise collaborate and coordinate with other professionals service providers and partners withinthe Behavioural Support Services;
- It is an expectation of the position that most of the work will be conducted in hospitals andcompliance with their occupational health polices and protocols will be required (associated costswill be reimbursed)
- Complete other duties from time to time as assigned by the Coordinator and/or Program
Director.
- Completion of a post-secondary educationin Social Services/Human Services or equivalent;
- Minimum three years related experience in providing support to vulnerable seniors and olderadults with responsive behaviours related to mental illness addictions dementia or otherneurological conditions;
- An understanding of harm reduction recovery and psychosocial rehabilitation principles thetherapeutic relationship and an ability to come to an agreement with clients on goals/priorities;
- Crisis intervention skills with well-developed problem solving abilities and interpersonal skills;
- Training/knowledge of P.I.E.C.E.S. U-First approaches or willingness to receive training;
- Experience/familiarity with RAI documentation and OCAN and health profession standards fordocumentation of client information
- Sound understanding and commitment to the principles of the Behavioural Supports Ontario(BSO) project and the Action Plan for the Central LHIN;
- Ability to collaborate effectively with other members of inter-professional care teams clientsfamily members and community team members;
- Good oral and written communication skills
- Computer literacy sufficient to produce and maintain files records or statistical information.
- Good analytical time management and organizational skills to contribute to program planningand to co-ordinate and follow-up on issues or activities with clients.
- Counselling and responding skills;
- Creativity and flexibility to approaches;
- Knowledge of community resources and supports;
- Ability to work effectively as part of a team and interact with workers from a variety of agencies;
- Strong assessment skills;
- Understanding of cultural competency and framework
- Valid drivers license and access to own vehicle is required;
- Staff will be required to work in residents rooms and may therefore be exposed to second handsmoke.
- May also be required to be on-call as emergency contact for after hours.
- Frequent travelwill also be required using own vehicle.
