Job Summary:
As a CaseManagerBehaviourSupports Ontario (BSO)you will be part of aBehaviourSupports Ontario (BSO)Team that provides direct care education and support to older adults and their carepartnerswhoare demonstrating responsivebehavioursthat may be associated with various types of dementia mental health substanceuseorother neurological will complete a comprehensivebehaviouralassessment and an evidence-basedBehaviouralSupport willassistin the creation and coordination of a support network through establishing linkages with other community agencies and practitioners to coordinate care for the person addition you will provide direct service to support the person served family members and/orcaregiversto mitigate escalating responsivebehaviours. You will also liaisecollaborateand communicate with allmembers of the BSO team and other partners within the circle of care.


The Behaviour Support Transition Resource Team (BSTR) provides direct care education and support to older adults and their care partners who are demonstrating responsive behaviours/personal expressions that may be associated with various types of dementia mental health substance use or other neurological conditions who are designated ALC or at-risk of becoming ALC in hospital settings. The team works within hospitals in Toronto York Region and South Simcoe and completes a comprehensive behaviour support assessment and creates a person-centered Behavioural Support Plan. Transition support is provided to those individuals who are being discharged from hospital to their next care destination. Serving over 700 individuals per year the Behaviour Support Transition Resource Team plays a key role in supporting hospital staff in the management of responsive behaviours/personal expressions.

What You Will Do:
  • Develop and implement individualized Behavioural Support Plans aimed at improving quality of life and reducing responsive behaviours.
  • Conduct behavioural assessments and maintain accurate timely documentation and reports.
  • Act as a liaison and advocate collaborating with clients families caregivers and multidisciplinary teams.
  • Provide training consultation and ongoing support to staff families and caregivers.
  • Offer supportive counselling and connect clients to appropriate health care services and community resources.
  • Deliver crisis intervention and assist in creating effective crisis management plans.
  • Build respectful trusting relationships using recovery-oriented and psychosocial rehabilitation principles.
  • Participate in team meetings care conferences and interdisciplinary collaboration.
  • Maintain strict confidentiality and adhere to organizational policies and procedures.
  • Work primarily within hospital settings ensuring compliance with health and safety protocols.
  • Engage in required training and contribute to the Behaviour Support Resource Team.
  • Perform additional duties as assigned by leadership.
What You Bring:
  • Post-secondary education in Social Services / Human Services or equivalent field.
  • Minimum 3 years working experience with client population group.
  • Understanding of challenges faced by seniors with mental health addictions dementia or neurological conditions.
  • Requires knowledge of recovery harm reduction recovery and psychosocial rehabilitation.
  • Knowledge of P.I.E.C.E.S. U-First RAI OCAN and documentation standards (or willingness to be trained).
  • Knowledge of community resources and cultural competency frameworks.
  • Proven crisis intervention problem-solving and interpersonal skills.
  • Strong assessment counselling and client engagement abilities with a flexible creative approach.
  • Ability to collaborate effectively with interdisciplinary teams clients families and community partners.
  • Excellent communication organizational and time management skills.
  • Proficient in documentation record-keeping and basic computer applications.
  • Valid drivers license and access to a personal vehicle.
Work Environment:
  • Staff will be required to work in clients rooms and may therefore be exposed to secondhand smoke.
  • Frequent travel within the service area using your own vehicle will also be required.
  • Occasional evening and weekend hours may be required.
