YOUR IMPACT

Diabetes Victoria is seeking a Peer Support Program Lead to coordinate strengthen and support the Diabetes Victoria Peer Support Program.

This role plays an important part in building the capacity of peer support groups across Victoria fostering relationships with community groups health professionals consumers and internal teams and supporting the sustainable growth of the peer support network.

Working within the Health Policy and Research department the Peer Support Program Lead will support the delivery of community-informed initiatives that empower people living with or affected by diabetes strengthen access to relevant information and resources and contribute to Diabetes Victorias broader access equity advocacy and health policy work.

Key Responsibilities:

  • Coordinate and support the Diabetes Victoria Peer Support Program network.
  • Build and maintain strong relationships with peer support groups health professionals consumers community groups and internal stakeholders.
  • Plan and conduct peer support group visits and maintain regular communication with the network.
  • Support the development quality reach and sustainability of peer support initiatives.
  • Promote Diabetes Victorias work campaigns resources events and membership opportunities to relevant community groups.
  • Respond to event and community engagement requests connected to the peer support network.
  • Contribute to access and equity initiatives including resource development reporting and evaluation activities.
  • Monitor and review peer support resources to ensure they remain current and relevant.
  • Use customer relationship management and feedback systems to monitor and report on peer support-related activities.
  • Prepare activity reports and contribute to continuous improvement across peer support programs and services.

ABOUT YOU

You will be a strong communicator and relationship builder who enjoys working withcommunities and supporting people to connect share information and access support. You will have experience working with community groups and be confident engaging with a range of stakeholders including consumers health professionals volunteers and internal teams.

You will bring strong organisational skills a practical approach to program coordination and an interest in supporting people living with or affected by diabetes or other chronic health conditions.

  • A tertiary qualification in health services community development health promotion or a related field.
  • Experience working with and supporting community groups.
  • Strong relationship-building stakeholder engagement and networking skills.
  • Well-developed written and verbal communication skills.
  • Experience in program coordination program implementation or community-based service delivery.
  • Strong customer service skills and the ability to respond respectfully and professionally to community enquiries.
  • Experience supporting events community activities or public engagement initiatives.
  • Ability to prepare review or edit written health-related information or community resources.
  • Understanding of evaluation reporting and continuous improvement principles.
  • Understanding of type 1 and type 2 diabetes or a similar chronic non-communicable disease would be highly regarded.

The successful candidate will be required to complete a current Victorian Police Check Working with Children Check and hold a valid drivers licence.
