About the OpportunityHASI Plus in Carlingford has opportunities for Mental Health Support Workers to join our team on a casual basis working across the following shift times:AM Shift: 6:30am 3:00pmPM Shift: 2:30pm 11:00pmAwake Overnight Shift: 10:30pm 7:00amThis is an opportunity to be part of a specialised recovery-focused program supporting people living with severe and persistent mental illness as they transition from long-term hospital mental health care into independent community living. Working within a shared model of care youll support consumers living in self-contained units across two sites helping them build confidence develop practical life skills and work towards the recovery goals that matter most to them. Every day is different with a balance of direct consumer support collaboration with the multidisciplinary team and opportunities to make a genuine impact on someones recovery day will include participating in handover reviewing the shared calendar completing medication observations alongside a colleague supporting consumers in the community and contributing to a safe supportive and recovery-oriented environment. Youll also benefit from regular team meetings operational supervision and group clinical supervision as part of a highly supportive team You In the RoleAs a Mental Health Support Worker youll work alongside consumers to support their individual recovery goals and promote independence choice and community participation. Supporting consumers to identify work towards and achieve their personal recovery consumers to attend appointmentsBuilding confidence with everyday living skills such as cooking meal planning budgeting and maintaining a participation in meaningful activities and supporting connections with local community groups services and social collaboratively with the multidisciplinary team to provide consistent person-centred accurate documentation to record progress outcomes and support medication observations in line with program Housing and Accommodation Support Initiative (HASI) PlusThis role is part of the Housing and Accommodation Support Initiative (HASI) Plus a statewide high-intensity transitional rehabilitation Plus supports adults aged 1865 living with severe mental illness and complex support needs who are transitioning from extended hospital care into the community.
