Job Summary

We are seeking a full-time or part-time Home Care Coordinator Enrolled Nurse to join our friendly local team based in Tumby Bay.

The Home Care Coordinator (Enrolled Nurse) is responsible for coordinating Home Care services and providing direct clinical care to consumers receiving support through the Commonwealth Home Support Programme (CHSP) Support at Home and the National Disability Insurance Scheme (NDIS). In this role you will:

  • Be a caring reliable and skilled professional who empowers consumers to make informed choices about their aged care and disability support services.
  • Work collaboratively with consumers through effective care management to achieve positive outcomes and ensure the delivery of high-quality clinical care and support services.
  • Promote and uphold the values culture and vision of West Coast Homecare contributing to a positive supportive and professional workplace environment.

Qualifications & Experience

  • Minimum of 3 years demonstrated experience working in the health aged care in-home care or disability support sectors.
  • Exceptional customer service skills with a strong consumer focus and proven ability to work effectively in a fast-paced environment while maintaining excellence in care and service delivery.
  • Flexible proactive and committed to completing tasks efficiently and within required timeframes.
  • Proficient in the use of computer systems including client management and other relevant software applications.

Compliance requirements

  • Diploma of Nursing current registration with NMBA.
  • Satisfactory NDIS Worker Check.
  • Current SA Drivers Licence essential.
  • Current First Aid & CPR certificate.
