Integral Home Care Pty Ltd (IHC) is an NDIS-registered disability support provider based in Blacktown NSW delivering support work nursing and transport services to participants across Greater Western Sydney. IHC is committed to person-centred trauma-informed and culturally safe care that supports participants to live independently and pursue their goals.
2. Position Summary
The Family Support Worker provides direct person-centred support to NDIS participants and their families in the home and community assisting with daily living activities personal care community access and family respite. The role works collaboratively with participants families and the broader care team to deliver support in line with each participants NDIS plan and individual support plan
upholding IHCs commitment to dignity safety and quality of care.
3. Key Responsibilities
- Provide direct support to participants and their families in accordance with individual support plans behaviour support plans (where applicable) and NDIS plan goals.
- Assist participants with activities of daily living including personal care meal preparation medication support (as trained and authorised) and household tasks.
- Support participants to access the community attend appointments and participate in social recreational and family activities.
- Provide in-home respite and family support to reduce carer stress and strengthen family capacity.
- Support the implementation of positive behaviour support strategies and de-escalation techniques where relevant to a participants plan.
- Maintain accurate timely and professional shift notes and incident reports via the organisations care management system.
- Report incidents hazards complaints and changes in participant wellbeing promptly in line with IHC policy.
- Work within a rostered shift pattern including flexibility for weekends evenings and public holidays as required.
- Maintain participant confidentiality and comply with NDIS Code of Conduct privacy legislation and IHC policies and procedures.
- Participate in supervision training and professional development including mandatory eLearning and competency requirements.
- Contribute to a safe working environment by following WHS procedures manual handling techniques and infection control practices.
4. Selection Criteria
Essential:
- Certificate III or IV in Individual Support Disability Community Services or a related field (or willingness to obtain).
- Current First Aid and CPR certification.
- Current NDIS Worker Screening Check (NDISWC) clearance or ability to obtain prior to commencement.
- Current Working with Children Check (if supporting participants under 18).
- Valid Australian drivers licence and access to a reliable insured vehicle.
- Sound understanding of person-centred practice the NDIS Code of Conduct and Practice Standards.
- Effective written and verbal communication skills including accurate case note recording.
- Ability to work independently and as part of a multidisciplinary care team.
- Legal right to work in Australia.
Desirable:
- Experience supporting participants with complex behaviours physical disability or high care needs.
- Training in positive behaviour support manual handling or medication administration.
- Experience working within an NDIS-registered provider environment.
5. Employment Conditions
- Employment is subject to the SCHADS Award 2010 and IHCs Employment Agreement and workplace policies.
- Ongoing employment is subject to satisfactory NDIS Worker Screening police check and reference checks.
- Completion of IHCs mandatory induction eLearning modules and required certifications prior to and during employment.
- This position may require travel between participant households; a vehicle allowance or kilometer reimbursement applies in line with the Award.

Expected Salary: $79500 AUD per annum (Excluding Super)
