Join our Leadership Team as a Clinical Outcomes Partner where youll play a pivotal role in driving clinical excellence quality improvement and funding optimisation across our residential aged care service. Working in close partnership with the Facility Manager and multidisciplinary teams you will lead clinical governance initiatives strengthen evidence-based practice and foster a culture of continuous improvement to deliver exceptional resident outcomes.
Key Responsibilities
- Provide clinical leadership to deliver person-centred evidence-based care aligned with the Aged Care Quality Standards legislation and organisational policies.
- Lead clinical governance quality improvement and compliance initiatives to enhance resident outcomes.
- Drive AN-ACC funding optimisation through accurate assessments documentation and funding compliance.
- Conduct clinical audits analyse data and implement quality improvement strategies.
- Develop and deliver clinical education programs to strengthen workforce capability and best practice.
- Promote WHS manual handling infection prevention and control and clinical risk management.
- Mentor and support nursing and care teams to foster a high-performing collaborative culture.
- Partner with the Facility Manager to support accreditation regulatory compliance and operational excellence.
- Collaborate with residents families and multidisciplinary teams to improve the resident experience.
- Contribute to leadership service improvement and provide operational support for the Facility Manager or Nurse Unit Manager when required.
About You
You are a passionate and experienced Registered Nurse with demonstrated success in aged care clinical leadership. You are committed to improving resident outcomes through strong clinical governance quality improvement and team development.
You will bring:
- Current AHPRA registration as a Registered Nurse.
- Demonstrated leadership experience within residential aged care.
- Strong knowledge of the Aged Care Quality Standards clinical governance frameworks accreditation requirements and aged care legislation.
- Experience with AN-ACC/ACFI funding clinical documentation and funding optimisation.
- Proven capability in quality improvement clinical auditing data analysis and compliance management.
- Experience designing and delivering clinical education coaching and mentoring programs.
- Excellent stakeholder engagement communication and change management skills.
- Strong understanding of risk management infection prevention and control medication safety and WHS principles.
- Ability to influence inspire and build high-performing multidisciplinary teams while fostering a positive resident-focused culture.
