Your Opportunity

The Case Worker role provides person-centred strengths-based support to young people aged 1024 with a focus on homelessness and early intervention. Working across multiple LGAs the role delivers assertive outreach and evidence-informed case management supporting clients to achieve sustainable housing build capacity and access appropriate services. The position requires strong relationship-building sound judgement and the ability to respond flexibly to the complex needs of vulnerable young people in community settings

Your key responsibilities will be to:

  • Work with and respond to referrals of clients across South West Sydney providing ongoing case management and support
  • Create individualized support plans including referrals to community servicesorganisations
  • Promotion of the service in the local community
  • Monitor our clients progress towards achieving their goals

Requirements for Success

  • Community Services Degree; or Associate Diploma or certificate with relevant experience;
  • Strong communication empathy and advocacy skills
  • Have a sound understanding of Mission Australias values and code of conduct
  • Well-developed written and oral communication skills including highly developed administrative skills
  • Current drivers licence.

Before starting work with us you will need to undertake a national police history check qualifications and referee checks and have a clearance to work with children/vulnerable people.
