About the role

We are seeking an experienced Social Worker to join our Grandcarer Case Management service. This initiative aims to support vulnerable Grandcarer families in navigating complex systems of care thereby enhancing the safety health and well-being of children and young people.

In this role you will build strong stakeholder relationships deliver workshops and events promote the Uplyft Grandcare Program and ensure Grandcarer voices inform advocacy and continuous improvement. The ideal candidate will provide high-quality person-centred support and help connect families with the services they need to achieve sustainable outcomes.

About you

You are a passionate and experienced Social Work professional who thrives in making a meaningful difference in the lives of families and children with complex needs. You bring a deep understanding of trauma-informed practice child development and family dynamics and you apply this knowledge with empathy cultural sensitivity and professionalism.

You hold a recognised Social Work degree and are eligible for membership with the Australian Association of Social Workers. Your background includes extensive hands-on experience in family casework particularly within diverse and vulnerable communities including Aboriginal families. You are skilled in conducting thorough assessments and providing thoughtful client-centered support.

You are a natural collaborator working effectively as part of a multidisciplinary team and are confident in guiding and supporting others including volunteers and students. Your strong administrative and organisational skills ensure smooth delivery of services and you are comfortable using a range of digital tools to manage your work. Youre also deeply committed to child safety and uphold the National Child Safe Principles in all aspects of your practice.

Ideally you bring experience in delivering training knowledge of government funding systems and perhaps even a background in evaluating human services or contributing to service development.

Most importantly you are an advocate at heart innovative driven and ready to champion the rights and needs of Grandcarers and the children they care for.
