Job Summary

You will provide case management services to families eligible to participate in the YMCA Caregiver Supports Program 
including supporting licensed and unlicensed caregivers of youth in foster care. Case management services include
coordinating or providing services to enable the child and caregivers to utilize medical educational social clinical
and other services necessary for maintaining the childs physical psychological and/or developmental health. You will
also provide leadership and support to the greater Case Management team and ensure that all clients in the Caregiver
Supports program are receiving quality case management services.

This position is with the Y Social Impact Center the social services division of the YMCA of Greater Seattle (YGS) serving around 5000 youth young adults and families every year in communities across King County Pierce County and various counties across WA. Through our programs we aim to strengthen young peoples ability to develop to their fullest potential through four strategies: reducing risk factors; navigating systems towards successful life transition; establishing stability safety and wellness in times of crisis; and building competencies and social capital. The Social Impact Centers programs include homelessness prevention housing behavioral health crisis response foster care licensing violence prevention and employment. Since many of the young people we serve have been involved in the foster care criminal justice behavioral health and/or homelessness systems we truly value relatable lived experience of our staff and those applying for our positions as an asset.

Hiring Range: $24.50 $27.50/hr

Schedule: Hybrid Monday-Friday; 8:30am-5:00pm with unpaid 30 minute lunch break and flexibility for evenings.

Responsibilities
  1. Provide case management and care coordination services as outlined in the Caregiver Supports Contract Statements of
    Work Handbooks and Policy/Procedures to clients enrolled in the Caregiver Supports Program including licensed and
    unlicensed caregivers of youth in foster care therapeutic foster care intensive in-home stabilization services and Outpatient
    Mental Health services.
  2. Maintain a caseload size in accordance with contractual requirements and guidelines.
  3. Ensure clientes receive all required screenings assessements services interventions and opportunities needed to fully meet their physical psychological social emotional relational developmental educational independent-living housing and permanency needs in a timely and effective manner.
  4. Develops individualized strengths-based youth-driven service plans.
  5. Must understand contract/grant terms and conditions to ensure program outcome requirements are met and services rendered to young adults appropriately.
  6. Researches best practices in field; creates and maintains specialized resource library.
  7. Maintains clear professional timely participant contact notes service plans progress updates outcomes and other documentation in designated databases.
  8. Teaches life skills job readiness or other subject matter individually and/or in groups often experientially.
  9. Teams effectively and collaboratively with YMCA staff and/or community partners to plan and facilitate activities including visits to related resources in the community.
  10. In coordination with the Program Director and in collaboration with the child & family team ensure clients transitions are well coordinated and facilitated including placements & discharges while ensuring that all required intake and discharge/exit documentation is completed in full on and on-time.
  11. Provide leadership and guidance in teh placement referral process and ensure that case transfers (of CMs Intervention Specliasts and Child & Family Therapists) are smooth and as lease disruptive as possible to the client.
  12. Builds and maintains collaborative relationships with funding liaisons other service providers businesses and/or educational institutions as related to specialized services.
  13. Supports participant success in key domains such as safe and stable housing permanent connections to supportive adults education and employment health and well-being and life skills.
  14. Ensure clients parent/child/sibling vistitation needs and requirements are met. Ensure natural supports and life-long connections are developed and stabilized.
  15. Ensure each clients file/record is fully in compliance with WAC DSHS/DLR DSHS/CA DSHS/DBHR KC-BHO COA Accreditation and YMCA requirements. Ensure all clients information is kept confidential and only released in accordance with HIPAA DSHS and YMCA policy and procedure.
  16. Conduct Intake Assessment and create individual service plans for non-BRS clients.
  17. Participate in clinical consults for clients as necessary in order to be able to provide clinical advocacy for conjunction with health professional assess clients mental health needs and determine appropriate level of care and support.
  18. May assist with program coordination or other activities related to job focus.
  19. Participates in YMCA annual fundraising campaign to engage community members in volunteerism and philanthropic support for the Y mission.
  20. Other duties as assigned.

Responsible for ensuring staff are properly screened trained supervised and held accountable to the organizations abuse risk management standards.

Qualifications
  1. Bachelors degree* in psychology social work or related social services field or equivalent experience Masters preferred. Other applicable education training and experience which provide the knowledge abilities and skills necessary to perform effectively in the position will be considered.
  2. Experience working with teens or young adults in employment foster care group care housing education juvenile justice anti-poverty vocational support mental health chemical dependency and/or homelessness fields.
  3. Ability to teach effective independence skills for a variety of learning styles and to plan supportive activities.
  4. Strong youth engagement and relationship-building skills.
  5. Strong verbal and written communication skills.
  6. Basic computer skills and working knowledge of Microsoft Office.
  7. Ability to transport self and client(s) in a safe and effective manner and to meet clients at community locations; proof of adequate vehicle insurance coverage is required or ability to travel to areas without public transportation.
  8. Current state approved First Aid and CPR certification.**
  9. Current approved HIV blood borne pathogens training.**
  10. Prefer knowledge of and previous experience with diverse populations (language culture race physical ability sexual orientation etc.). Ability to speak any language in addition to English may be helpful.

Preferred Qualifications:

A Masters degree in social services or a closely related field; OR A Bachelors degree in social services or closely related field; OR Five (5) years full-time of documented experience working with children or youth in one of or a combination of the following settings:  licensed in-home or facility-based daycare or professional nanny;  setting or early learning center;  Health Hospital;  Rehabilitation facility detention center law enforcement or corrections; v.A child placing agency residential care program or as a licensed foster parent;  supervised visits; or  as staff directly with children or families in a community service organization. Staff with a bachelors degree must consult with a person with a masters degree in social services or closely related field for one (1) hour for every twenty (20) hours the Support management employee works. 

Authorized Driver Requirement

This role transports clients as an authorized driver for the YMCA of Greater Seattle (YGS) using an employer-provided vehicle and using your own vehicle (proof ofcurrent auto insurance and vehicle registration are). Driving duties for this role areRequired.*Driving roles aresafety-sensitive. To become an authorized driver you must be 21 havepossesseda drivers license for at least 3 years and pass a drug screen that will test for impairing substances (including marijuana) in addition to meeting the driving record standards below:

YGS Driving Record Standards

Within the last three (3) years: No more than two (2)vehicle relatedsuspension(s) and/or reinstatement(s). No more than two (2) moving violations and/or accidents. Moving violations include but are not limited to the following:

  • Speeding
  • Improper Lane Change
  • Failure to Yield in Right-of-Way
  • Failure to Obey Traffic Signal
  • Failure to obey Traffic Sign

Within the last five (5) years: No Major Violations which include but are not limited to the following:

  • DUI DWI OUI OWI BUI (Boating) Administrative Per Se or Refusing to Test
  • Driving with an open container of alcohol
  • Reckless Driving
  • Hit andRun property damage only (Incidents involving injury fall under Permanent Disqualification)
  • Racing
  • Driving with a suspended/revoked license/insurance
  • Speeding over 20 mph
  • Fleeing and/or eluding police
  • Any other felony driving conviction(s)

Driving records that result in Permanent disqualification from driving duties:

  • More than 3 DUI DWI OUI OWI BUI Administrative Per Se or Refusing to Test forentiretyof driving history.
  • More than 3 Driving with a suspended/revoked license
  • Hit andRunresulting in bodily injury or death
  • Manslaughter/felony death by motor vehicle
