Job Responsibilities:
- Discuss and review patients medical history symptoms allergies and current medications
- Perform comprehensive physical examinations and clinical assessments
- Ask patients situation-specific questions to formulate accurate diagnoses
- Diagnose and treat acute and chronic medical conditions
- Develop and implement individualized treatment plans focused on improving patient outcomes
- Prescribe suitable medications and provide proper dosage and administration instructions
- Order review and interpret laboratory imaging and diagnostic test results
- Provide preventive care services wellness visits health screenings and patient education
- Collaborate with care teams and specialists to coordinate comprehensive patient care
- Manage patient panels and support value-based care initiatives quality measures and population health goals
- Maintain accurate and timely documentation in eClinicalWorks (eCW)
Job Qualifications:
- Doctor of Medicine (MD) Nurse Practitioner (NP) or Physician Assistant (PA)
- Board-certified in Family Medicine Internal Medicine or a related field
- Active Arizona license or ability to obtain prior to start date
- Experience providing primary care services in an outpatient setting preferred
- Sound medical knowledge
- Experience utilizing electronic medical record systems preferably eClinicalWorks (eCW)
- Knowledge of value-based care models quality measures and population health management preferred
- Excellent analytical and problem-solving skills
- Exceptional communication skills
- A patient and compassionate disposition
- Detail-oriented
