Job Responsibilities:

  • Discuss and review patients medical history symptoms allergies and current medications
  • Perform comprehensive physical examinations and clinical assessments
  • Ask patients situation-specific questions to formulate accurate diagnoses
  • Diagnose and treat acute and chronic medical conditions
  • Develop and implement individualized treatment plans focused on improving patient outcomes
  • Prescribe suitable medications and provide proper dosage and administration instructions
  • Order review and interpret laboratory imaging and diagnostic test results
  • Provide preventive care services wellness visits health screenings and patient education
  • Collaborate with care teams and specialists to coordinate comprehensive patient care
  • Manage patient panels and support value-based care initiatives quality measures and population health goals
  • Maintain accurate and timely documentation in eClinicalWorks (eCW)

Job Qualifications:

  • Doctor of Medicine (MD) Nurse Practitioner (NP) or Physician Assistant (PA)
  • Board-certified in Family Medicine Internal Medicine or a related field
  • Active Arizona license or ability to obtain prior to start date
  • Experience providing primary care services in an outpatient setting preferred
  • Sound medical knowledge
  • Experience utilizing electronic medical record systems preferably eClinicalWorks (eCW)
  • Knowledge of value-based care models quality measures and population health management preferred
  • Excellent analytical and problem-solving skills
  • Exceptional communication skills
  • A patient and compassionate disposition
  • Detail-oriented
