As a Home Health Occupational Therapist youll help patients regain the skills strength and confidence they need to safely perform everyday activities while remaining in the comfort of their own homes.

- Complete occupational therapy evaluations reassessments and discharge visits
- Develop individualized treatment plans based on patient goals and functional needs
- Provide skilled occupational therapy services in patients homes
- Assess home environments and recommend adaptive equipment and safety modifications
- Educate patients and caregivers on home safety adaptive equipment energy conservation techniques and activities of daily living
- Monitor patient progress and modify treatment plans as appropriate
- Collaborate with physicians nurses and the interdisciplinary care team
- Complete timely and accurate clinical documentation using WellSky
- Active Maryland Occupational Therapist License Required
- Valid Drivers License and Reliable Transportation
- Strong Clinical Assessment Communication and Documentation Skills
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with WellSky Home Health is a plus
