Position Summary
- Oversee transitional housing opportunities for individuals diagnosed with a severe and persistent mental illness.
- Review clinical information determine level of care authorize treatment.
- Work on a team to actively coordinate ongoing client care and discharge planning to transition to less restrictive settings.
- Weekly case staffing with providers and clients.
- Daily collaboration in a fast-paced team-centered environment.
What We Value in our next Triage Clinician
- Bachelors or masters degree in counseling social work or other mental health related field; and current Ohio licensure. LSW LISW LISW-S LPC LPCC and LPCC-S are all desirable.
- Clinical assessment skills understanding diagnostic criteria and symptom presentations outline in the DSM-5.
- Working knowledge of the Hamilton County behavioral health system of care preferred.
- A strong alignment with trauma-informed person-centered care.
