Travel Social Worker

Rural hospital located several hundred miles northwest of Anchorage AK seeking a full-time Travel Social Worker to join their team.

Pay: $60.00/hour

Travel Social Worker Opportunity:

• Provide medical social work services to patients of all ages including the patients located in the Acute Care Unit Primary Care Clinic
  
• Respond to requests for medical social work services including case management discharge planning nursing home/assisted living placements risk man agreement crisis intervention and care coordination
  
• Social Worker Coordinate nursing home and assisted living placements for patients.
  
• 6-month contract
  

Travel Social Worker Qualifications:

• Must be a licensed Social Worker
  
• Performing medical social work child protection or adult protection services or related field
  
• Ability to use and implement the psycho-social diagnostic process of assessment diagnosis planning intervention and evaluation
