We are seeking a dependable compassionate organized and safety-focused Transportation Driver / Central Supply Coordinator to join our skilled nursing team.
This unique dual role is ideal for someone who enjoys working directly with residents while also having strong organizational and administrative skills. You will play an important part in the resident experience by providing safe and comfortable transportation to medical appointments and community destinations while also supporting the facility through the accurate organization maintenance and coordination of our central supply stock.
Youll work closely with nursing leadership clinical staff hospitals physician offices and other healthcare providers to help ensure residents receive coordinated timely and compassionate care.

- Coordinate and transport residents safely to and from medical appointments outpatient programs hospitals and community facilities.
- Develop and follow a daily transportation schedule based on resident needs and appointment priorities.
- Coordinate the transportation of new admissions and returning residents from hospitals and other healthcare facilities.
- Communicate professionally with hospitals physician offices clinics and outside providers regarding appointment and transportation needs.
- Observe residents during transportation and promptly report any changes in condition or concerns to nursing staff.
- Assist residents with safe transfers mobility and positioning as directed by the clinical team.
- Ensure residents have necessary belongings paperwork and information prior to leaving for appointments.
- Notify nursing leadership of appointment changes delays cancellations or transportation concerns.
- Provide hydration and help ensure resident comfort throughout transportation.
- Maintain the facility vehicle in a clean safe and professional condition.
- Complete required vehicle inspections and promptly report maintenance or safety concerns.
- Protect resident belongings and maintain professionalism and confidentiality at all times.
- Maintain resident medical records in an accurate organized complete and confidential manner.
- Assist with filing scanning uploading indexing and organizing resident clinical documentation.
- Review medical records for missing incomplete or improperly filed documentation and follow up with the appropriate department or team member.
- Assist with maintaining both electronic and paper medical records according to facility policies and procedures.
- Process authorized requests for medical records and ensure information is released only to appropriate individuals or organizations.
- Assist with obtaining hospital records physician documentation laboratory results diagnostic reports and other clinical information as needed.
- Prepare and organize necessary medical documentation for resident appointments and outside consultations.
- Ensure documentation received following resident appointments is routed to the appropriate nursing or clinical team member.
- Assist with organizing records for new admissions hospital returns transfers and discharges.
- Support nursing leadership with medical record audits and documentation follow-up.
- Track outstanding records and communicate with outside providers to obtain needed documentation.
- Maintain resident confidentiality and safeguard protected health information in accordance with facility policies and applicable privacy requirements.
- Work collaboratively with Nursing Admissions Social Services Therapy and other departments to support accurate and timely resident documentation.
- Complete other medical records transportation and administrative duties as assigned.
- Certified Nurse Aide (CNA) in good standing required
- Successful completion of a State-Approved Nurse Aide Training Program
- Valid drivers license in good standing
- Current BLS/CPR certification required
- Previous experience in a skilled nursing long-term care healthcare or medical records setting preferred
- Strong organizational skills and attention to detail
- Basic computer skills and ability to work with electronic ordering platorms or other healthcare purchasing/ordering systems
- Ability to manage multiple priorities schedules and deadlines
- Strong written and verbal communication skills
- Ability to maintain confidentiality and handle sensitive resident information appropriately

- Ability to work independently while also being an effective member of the interdisciplinary team
This is more than a driving position. You will be an important connection between our residents their care team and outside healthcare providers.
