Job Summary
JOB SUMMARY:
The Torch Hub Anchor Program Coordinator serves as the operational lead for Missouris Rural Health Transformation Program (RHTP) within the Transformation of Rural Community Health Care (ToRCH Care) model. Based within a designated Hub Anchor organization this position coordinates daily Hub operations by bringing together healthcare providers public health agencies emergency services behavioral health organizations schools and community-based partners to improve access to coordinated clinical behavioral and social services for rural residents. The coordinator oversees local program implementation facilitates Leadership Board activities monitors Hub performance supervises RHTP Navigator workflows strengthens referral and care coordination processes and supports continuous improvement using data dashboards and Community Information Exchange (CIE) tools.
The Torch Hub Anchor Program Coordinator reports directly to the Population Health Director.
QUALIFICATIONS:
- Education:
- Associate degree or equivalent combination of education and relevant experience is required.
- 13 years of experience in healthcare operations community health care coordination public health quality improvement or a related field is required.
- Licensure:
- Valid Drivers License
- Work Experience:
- Experience in community health work case management patient navigation Medicaid care management behavioral health support public health outreach social services or similar settings.
- Demonstrated ability to coordinate multi-stakeholder initiatives involving healthcare providers government agencies and community organizations.
- Experience building partnerships with hospitals FQHCs Rural Health Clinics EMS behavioral health providers public health agencies schools and community organizations.
- Strong analytical organizational and problem-solving skills.
- Experience using data dashboards reporting tools and technology platforms to support operations and performance improvement.
- Knowledge of care coordination workflows social determinants of health (SDOH) community resource navigation and closed-loop referral systems.
- Experience serving rural communities or high-need populations facing barriers such as transportation housing food insecurity or limited healthcare access.
- Strong written and verbal communication skills with the ability to build collaborative relationships.
RESPONSIBILITIES/JOB DUTIES:
- Serve as the primary HUB spokesperson
- Convene Hub partners to assess community needs establish priorities and implement local strategies.
- Coordinate and oversee implementation of local and statewide RHTP initiatives.
- Monitor Hub performance and report operational progress risks and improvement opportunities.
- Supervise RHTP Navigator workflows to ensure effective care coordination and closed-loop referrals.
- Facilitate Leadership Board meetings prepare agendas document decisions and track action items.
- Coordinate onboarding and engagement of healthcare providers and community partners.
- Develop and improve referral pathways communication processes and operational workflows.
- Collect analyze and report operational and performance data using CIE systems and dashboards.
- Identify service gaps workflow barriers and operational challenges while coordinating improvement efforts.
- Maintain active community engagement through meetings outreach and partnership development.
- Support readiness activities for new programs including workflow testing and implementation planning.
- Collaborate with Regional Coordinating Networks (RCNs) and Rural Health Transformation Office (RHTO) to achieve Hub goals.
- Perform other related duties as assigned.

WORKING CONDITIONS:
- Full-time position generally working Monday through Friday 8:00 a.m. to 5:00 p.m. with occasional evening or weekend meetings and community outreach.
- Frequent travel throughout the assigned rural Hub service area.
- Primarily office-based within a Hub Anchor organization with regular work conducted in hospitals clinics public health agencies schools EMS locations pharmacies and community organizations.
- Frequent interaction with healthcare professionals community leaders public agencies and partner organizations.
- Extensive use of computers electronic referral systems dashboards databases and communication technologies.
- Requires the ability to manage multiple projects deadlines and competing priorities in a collaborative environment.
- Position requires regular attendance at community meetings stakeholder events and leadership sessions.
- Successful completion of required background screening is a condition of employment.
