Summary
Provides direct services initiates and carries out individual client-related treatment plans and liaison services with the inpatient and outpatient programs and other community services providers.

Job Duties

• Prepares for a clients discharge from the program by developing a client-specific discharge plan upon program entry and makes community referrals.
• Provides direct instruction of activities of daily living in accordance with program county and state regulations.
• Reports all maintenance issues to leadership or designee according to policy.
• Maintains program building and client apartments according to set policies and regulations.
• Participates in program performance improvement activities.
• Designs and implements individual treatment plans for clientele.
• Ensures clientele is receiving appropriate financial resources and coordinates same.
• Assists program nurse in client medication observation and documentation.

Minimum Qualifications

• High School Diploma/GED
• 4 years within mental health setting.

Preferred Qualifications

• Bachelors Degree in Human Services.

Physical Demands
Lift and carry 40-50 lbs. Examples: Push/pull patients on bed stretcher (requires 29 lbs. push force) lateral transfers up to 50 lbs. of the patients weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling bending squatting and performing CPR.
