JOB DESCRIPTION
POSITION TITLE: Supportive Housing Case Manager
EXEMPTION STATUS: Exempt
REPORTS TO: Supportive Housing Program Manager
POSITION DESCRIPTION: Provides intensive housing and health case management services including home visits to clients homes using a harm reduction approach through a permanent supportive housing program to a diverse set of formerly homeless individuals who are impacted by chronic medical issues
