Job Summary

Position Summary

Under the supervision of the Project Manager the Viral Hepatitis Case Manager provides patient-centered support to help clients overcome barriers to hepatitis treatment HIV linkage to care and access to needed services. This role coordinates referrals supports continuity of care conducts outreach and contributes to program development staff training and quality improvement efforts.

Key Responsibilities
  • Provide case management for patients with viral hepatitis from linkage to care through treatment completion.
  • Coordinate referrals and support access to medical social and community resources.
  • Conduct outreach and linkage-to-care services at community screening events and partner sites.
  • Support patients needing both hepatitis and HIV care coordination.
  • Develop and maintain patient tracking systems and referral workflows.
  • Assist with staff training resource development and program implementation.
  • Reconcile patient support fund expenses including transportation and medication assistance.
  • Collaborate with hospitals community agencies and addiction treatment centers on education and outreach.
Qualifications
  • Bachelors degree in human services or related field required.
  • Minimum of 5 years of relevant experience required.
  • Strong knowledge of viral hepatitis HIV addiction services and care coordination.
  • Excellent communication organizational and interpersonal skills.
  • Ability to work independently build community partnerships and provide compassionate nonjudgmental support.

Salary Range: Minimum 19.25/hour - Maximum 31.80/hour. Compensation will be commensurate with equity and experience for roles of similar scope and cases where the range is displayed as a $0 amount salary discussions will occur during candidate screening calls before any subsequent compensation discussion is held between the candidate and any hiring authority.
