Are you passionate about supporting the social and psychological well-being of older adults and their families Trinity Grove is seeking a dedicated and compassionate Social Worker to join our Social Work Department and play a vital role in our community.

As a Social Worker at Trinity Grove you will be responsible for ensuring the emotional social and psychological well-being of our residents and their families. Reporting to the Administrator you will work closely with our care team to provide support advocate for residents rights and contribute to a positive living environment.

• Support the social and psychological health of residents and families.
• Conduct in-service sessions on important topics such as residents rights advanced directives and psychological well-being.
• Serve as a resource and advocate for residents and families navigating transitions and challenges.
• Collaborate with interdisciplinary teams to ensure holistic care.
• Maintain records and documentation in compliance with organizational policies.

• Bachelors or masters degree in social work or related field required.
• Relevant licensure as required by state regulations.
• Experience working with older adults or in a healthcare setting preferred.
• Strong communication organizational and interpersonal skills.
• Commitment to upholding residents rights and promoting their well-being.
